We provide relief and assurance to family members in the areas of In‑Home Personal Care, Alzheimer’s & Dementia Care, Palliative & Hospice Care, and Geriatric Care Management. Details
  • Clients throughout the five boroughs requiring shifts from 8 hrs to 12 hrs.
  • Day and night shifts available.
  • Opportunities to make skilled nursing visits.
  • Work full‑time or around your current schedule.
  • Shifts are assigned to provide 1:1 care in the home environment.
Qualifications
  • NYS RN licensure.
  • BLS/CPR/ACLS/PALS certification.
  • Minimum of one year demonstrated experience as an RN.
  • Ability to put patients at ease and explain physician's instructions.
  • Excellent organizational and multitasking skills.
  • Experience working with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player able to work independently.
  • Valid driver's licence or NYS Non‑Driver photo  required.
Essential Duties & Responsibilities
  • Assess client needs and develop a plan of care.
  • Consult with management on complex or unique cases.
  • Provide a plan of care with support of the clinical/medical and multidisciplinary members to ensure services follow the plan of care by assessment, planning, coordination, counseling, teaching, and supervision.
  • Monitor and assess patient/client condition and effects of the care plan.
  • Oversee/supervise the care provided to patients by Licensed Practical Nurses and paraprofessionals.
  • Manage, plan, coordinate, and execute the patient's safe discharge from service.
  • Ensure physician's orders, discharge summary, and communication to team members, patients, and family members adhere to company and regulatory requirements.
  • Communicate and consult with patient's MD as needed, managing and coordinating all aspects of the assigned caseload.
  • Assist patients with activities of daily living.
  • Document content and process of all services provided in the internal system properly.
  • Perform other related duties and participate in special projects as assigned.
