Cardiology Nurse Practitioner

We re hiring a full-time advanced practice clinician to join a growing post-acute care team in the Brooklyn, New York area.

This position focuses on delivering high-quality, compassionate cardiology consults to adult patients in skilled nursing settings. With a Monday through Friday schedule and no nights or weekends, this role supports both your professional goals and your personal well-being.

• Perform cardiology consults in post-acute care environments
• Monitor and manage chronic cardiac conditions in collaboration with facility teams
• Work closely with supervising physicians and interdisciplinary staff
• Contribute to quality improvement efforts and patient safety initiatives
• Document using a mobile EMR system, with full training and technical support provided

• Certified nurse practitioner with an active New York license
• Active DEA registration (or ability to obtain)
• At least 2 years of experience in cardiology or post-acute care preferred
• Compassionate and collaborative, with a patient-first mindset
• Open to additional cardiology training if needed
