Per Diem LPN

This is a 13-week contract position.

MUST be able to start full-shift orientation on a Tuesday or Thursday. Weekends and holidays are required.

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care to residents, predominantly within adult and geriatric populations. This role requires adherence to defined policies and procedures and effective collaboration with other nursing staff, physicians, and ancillary team members.

Responsibilities:

• Collect and assess data relevant to each resident's physical and psychosocial condition, communicating findings to the Registered Nurse (RN) and interdisciplinary team.
• Contribute to the creation of age-appropriate and culturally relevant care plans, actively involving residents and families in the process.
• Recommend revisions to care plans based on resident outcomes and needs, ensuring all aspects of care are adequately addressed.
• Maintain and enhance current knowledge and skills in nursing practice through ongoing education and in-service programs.
• Provide leadership under the supervision of the RN and participate in mentoring or orienting new staff.
• Demonstrate awareness of safety and cost considerations in care delivery.

Required Experience / Certifications / Licensure:

• Current NY LPN Licensure.
• BLS certification through AHA/RQI.
• Minimum of 1 year of experience as an LPN in a long-term care facility.
