Duties & Responsibilities:
  • Provide nursing care to individuals in accordance with their assessed medical needs,physician orders, nursing plans, and agency procedures.
  • Monitor individuals for changes in physical or medical condition and promptly communicate significant changes to the appropriate Registered Nurse, medical provider, supervisor, and/or residential management.
  • Perform nursing evaluations and follow established protocols for health concerns,injuries, illnesses, and changes in condition.
  • Provide treatments and other nursing interventions within the scope of practice and in accordance with applicable orders.
  • Monitor individuals with chronic medical conditions and ensure appropriate follow-upand documentation.
  • Recognize and respond appropriately to medical emergencies and follow agency emergency procedures, including contacting emergency medical services when indicated.
  • Participate in health and safety reviews and assist with identifying potential healthrisks.
  • Follow infection prevention and control practices according to agency protocol and procedures.
  • Coordinate and/or support individuals' medical, dental, psychiatric, specialty, and other healthcare appointments as assigned.
  • Review appointment recommendations and ensure that follow-up orders and instructions are communicated to the appropriate members of the individuals’ care team.
  • Assist with obtaining and maintaining medical documentation from healthcare providers.
  • Followup on laboratory tests, diagnostic studies, referrals, treatments, and other medical recommendations.
  • Ensure that medical recommendations are communicated and implemented in a timely manner.
  • Document appointments, recommendations, follow-up actions, and outcomes in the appropriate record
  • Communicate relevant medical information,changes in condition, and outstanding follow-up needs to the appropriate nursing and residential staff
  • Complete all required charting in the agency's designated Electronic Medical Record(EMR) regarding the medical care and health status of individuals served,including medical concerns, interventions, changes in condition,communications, follow-up, and other pertinent medical information.
  • Maintain and update the individual's paper medical chart in accordance with agency policies, nursing standards, and applicable regulatory requirements.
  • Ensure required medical documentation is accurately maintained in the appropriate location, whether in the EMR or paper medical chart.
  • Complete and/or ensure completion of routine medical documentation and tracking requirements
  • Assistby reinforcing established training to Direct Support Professionals and other staff regarding individuals' specific medical needs and health-related supports.
  • Maintain appropriate medication storage, security, accountability, and disposal practices.
  • Assist with medication reconciliation and ensure that medication orders and documentation are current.
  • Participate in medication reviews and audits assigned.
  • Ifneeded, administer medications safely and accurately in accordance with physician orders, the individual's medication regimen, agency policies, and applicable regulations and document as required
  • Perform other nursing-related duties within the scope of practice as assigned.
  • Other related duties as assigned.
Education & Experience

Current New York State Licensed Practical Nurse (LPN) license in good standing.

Ability to provide nursing services in accordance with the LPN scope of practice.

I year Experience working in an OPWDD-certified residential program or similar community-based setting preferred

Current Basic Life Support (BLS) certification from a recognized provider, maintained throughout employment.

Physical Requirements:
  • Maybe required to escort individuals to medical appointments
  • Prolonged periods of sitting at a desk and working on a computer.
  • Some physical effort when assisting or providing care to individuals
  • Maybe required to lift up to 25 pounds or more.
  • Required to commute to other agency locations as needed.
