Position: Infectious Disease NP/PA — On-Site Care Leader (Flexible)
Location: New York

Elaya Health is seeking a Nurse Practitioner or Physician Assistant to join our Infectious Disease division, delivering on-site  in skilled nursing and long-term care facilities. You will evaluate and manage a wide range of infections with strong collaboration with facility teams and administration.

This role offers flexible scheduling, strong physician and administrative support, and significant clinical autonomy as the program grows within our expanding specialty division.
