• Administer medications and treatments as ordered

• Monitor residents and communicate changes in condition

• Complete accurate and timely documentation in Matrix Care

• Communicate with physicians, families, and the interdisciplinary team

• Supervise and support CNAs on the unit

• Help maintain a safe, comfortable, and positive environment for our residents

• Current New York State LPN license

• Compassionate, dependable, and professional

• Strong clinical and communication skills

• A team player with a resident‑first mindset

• Skilled nursing or long‑term care experience preferred

• Matrix Care experience preferred
