Full Time, 5x8s with rotating shifts including Days, Evenings, Nights, and Weekends. Holiday work is required.

1 year of experience as a Licensed Practical Nurse required.

• Current New York LPN Licensure
• BLS certification (AHA or RQI only)

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and the interdisciplinary team.
• Contribute to developing age-appropriate and culturally relevant resident care plans, involving the resident/family and healthcare team.
• Evaluate resident outcomes and recommend revisions to care plans as necessary, collaborating with residents/families and the interdisciplinary team.
• Maintain competency in nursing practice and participate in the orientation and mentoring of staff.
• Demonstrate knowledge and clinical expertise in caring for geriatric residents.
