Full Time D/N rotation (5x8s). EOW and Holiday required.
1 year of experience as an LPN required.
Current New York LPN Licensure
BLS certification (through AHA or RQI specifically)
Provide direct and indirect skilled nursing care for assigned residents.
Collect and communicate data relevant to residents' physical and psychosocial conditions.
Contribute to the development of culturally relevant care plans in collaboration with the healthcare team.
Evaluate resident outcomes and recommend revisions to the plan of care as necessary.
Maintain knowledge and competency in nursing practice through continuing education.
Participate in mentoring and orientation of new staff.
