Licensed Practical Nurse (LPN) - Full-Time Evenings

This is a 13-week contract position.

Full-Time evenings, 5x8s. All contracts must run to February/March end dates and start on a Tuesday or Thursday.

The Licensed Practical Nurse (LPN) will provide essential nursing care appropriate to the age of the patients served. The individual will demonstrate knowledge of principles of growth and development throughout the life span, and possess the ability to assess data reflective of the patient's status and interpret appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.

This role involves providing direct and indirect skilled nursing care under the supervision of a Registered Nurse.

Responsibilities:

• Continuously collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and the Interdisciplinary Team.
• Contribute to the creation of a developmentally age-appropriate and culturally relevant care plan with nursing diagnoses and interventions.
• Recommend revisions to the care plan based on evaluations of resident outcomes and initiate referrals as needed.
• Maintain current knowledge and clinical competency while participating in mentoring and orientation of new staff.
• Provide leadership in the professional practice setting under the direction of the RN.

Required Experience / Certifications / Licensure:

• Current NY LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year experience serving in an LPN capacity at a Long-Term Care Facility
