Nurse Practitioner – HIV Primary Care & Behavioral Health

Are you an experienced Nurse Practitioner interested in combining primary care, HIV medicine, and behavioral health within a mission-driven community healthcare setting?
A New York City healthcare organization is expanding its integrated medical and behavioral health services and is seeking an experienced Nurse Practitioner to help support a new initiative focused on providing comprehensive care to adults living with HIV and other complex healthcare needs.
This is a unique opportunity to help build and grow a new clinical program while providing hands-on care to an underserved adult patient population.
The OpportunityThe Nurse Practitioner will primarily provide primary and preventive care with a strong focus on HIV treatment and management
, while working closely with psychiatric, behavioral health, substance-use, and residential treatment teams.
You'll care for adult patients ages 18+ and help coordinate both their physical and behavioral healthcare needs.
The position will work across locations in the Bronx and Harlem
.
Responsibilities will include:
- Provide comprehensive primary care to adult patients
- Evaluate, diagnose, and treat acute and chronic medical conditions
- Provide ongoing care and treatment for patients living with HIV
- Prescribe and manage medications within scope of practice
- Monitor treatment effectiveness, medication adherence, and potential side effects
- Order and interpret laboratory and diagnostic testing
- Provide preventive care, health screenings, and patient education
- Help patients understand and manage chronic medical conditions
- Support medication adherence and long-term HIV care
- Identify behavioral health and substance-use needs that may impact a patient's overall health
- Collaborate closely with psychiatric and behavioral health providers
- Coordinate care for patients with co-occurring medical, psychiatric, and substance-use conditions
- Work with multidisciplinary teams across outpatient, residential, and community-based programs
- Coordinate referrals to specialists, hospitals, and outside healthcare providers when appropriate
- Support continuity of care as patients transition between different healthcare settings
- Maintain accurate and timely clinical documentation
- Participate in clinical quality and performance-improvement initiatives
- Contribute to the development and growth of a new integrated healthcare initiative
You’ll work with adults ages 18+, including individuals with complex medical and behavioral health needs.
The patient population may include individuals receiving care for:
- HIV
- Chronic medical conditions
- Mental health conditions
- Substance-use disorders
- Medication-assisted treatment
- Co-occurring medical and behavioral health conditions
This position is particularly well suited for a provider who believes physical health, mental health, and substance-use treatment should be approached as part of one coordinated care plan.
What We're Looking For- Active New York State Nurse Practitioner license
- Family Nurse Practitioner (FNP) or other appropriate primary-care NP background
- Experience providing adult primary care
- Experience treating patients living with HIV strongly preferred
- Comfortable prescribing and managing medications
- Strong chronic disease-management experience
- Experience in community health, infectious disease, HIV medicine, behavioral health, substance-use treatment, or similar settings is highly desirable
- Comfortable treating medically and behaviorally complex patients
- Strong clinical assessment and decision-making skills
- Comfortable working within a multidisciplinary care model
- Interest in helping develop and grow a new clinical program
