Nursing Coordinator, RN Nurse

Responsibilities

• Assist all patients through the health care system by acting as a patient advocate and navigator.
• Participate in Patient Centered Medical Home team meetings and quality improvement initiatives.
• Facilitate health and disease patient education, including leading group office visits.
• Support patients’ self-management of disease and behavior modification interventions.
• Coordinate continuity of patients and families following hospital admission, discharge, and ED visits.
• Conduct comprehensive, preventive screening for patients and assist all support staff in daily patient interactions as needed.
• Promote clear communication among a care team and treating clinicians by ensuring awareness regarding patient care plans.
• Facilitate patient medication management based upon standing orders and protocols.
• Participate on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Patient Centered Medical Home and Medical Neighborhood initiatives.
• Evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
• Be a team player that can follow a system and protocol to achieve a common goal.
• Be highly organized and have well-developed oral and written communication skills.
• Demonstrate sound judgement, decision-making and problem solving skills.
• Maintain confidentiality in accordance with practice, State and Federal regulations.
• Communicate and outreach to other community health care organizations and personnel.
• Perform all other duties as required.

Formal Education and Job-Related

Experience:

High School Diploma required.

Desired (not required) criteria: This position requires a minimum formal education of Bachelor's Degree and a minimum of 5 years job-related experience.

License, Registration, or Cert.

Required:

LPN or RN New York City License Preferred.
