MDS Assessor

Location: Ditmas Park, Brooklyn, NY

Schedule: Monday–Friday, 9:00 AM–5:00 PM

Salary: $120,000–$130,000 per year

Ditmas Park Nursing & Rehab is seeking an experienced and detail-oriented MDS Assessor to join our clinical team.

The MDS Assessor will be responsible for completing accurate and timely resident assessments, supporting individualized care planning, and ensuring compliance with federal and state regulations.

• Complete MDS assessments accurately and within required time frames
• Review resident documentation and clinical records
• Coordinate with nursing, therapy, social services, dietary, and other departments
• Participate in interdisciplinary care-plan meetings
• Ensure assessments reflect each resident’s condition and level of care
• Monitor documentation for accuracy, consistency, and compliance
• Support proper reimbursement under the Patient-Driven Payment Model (PDPM)
• Maintain compliance with CMS and New York State requirements

• Active New York State RN license
• Previous MDS experience in a skilled nursing or long-term-care facility required
• Strong understanding of MDS 3.0, PDPM, and care planning
• Excellent organizational and documentation skills
• Ability to meet deadlines and work collaboratively with an interdisciplinary team
