MJHS invites applications for a Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments of potential hospice patients in home, hospital, or facility settings. This role will develop individualized care plans, set goals, and make recommendations to meet patient needs.
You will admit eligible patients to the Hospice program in line with NHPCO standards, requiring a current NYS RN license and a NYS driver’s license.
