- Hospice (Field) The Field Licensed Practical Nurse provides direct patient care to hospice patients in a variety of settings and levels of care under the direct supervision of an RN. Days/Nights/Weekends/Holidays as assigned to provide 24 hours a day, 7 days a week care assigned to a variety of teams/duties.

• Providing care to the terminally ill patient
• Reinforcing patient and family training and education
• Performing patient assessment and documentation
• Participating in the comprehensive Interdisciplinary Plan of Care
• Participating in the Quality Improvement activities

All services will be provided 24 hours a day, 7 days a week as assigned to a variety of teams and duties. Nights and weekends. On-Call rotations as assigned.

• Valid Driver's license with a good driving record
• Personal car in good working order
• Valid Florida license for Licensed Practical Nurse (LPN) BLS and IV certification per department requirement
• Minimum 1 year experience in Medical Surgery Nursing or equivalent Hospice experience preferred
• Graduated from an accredited nursing program, must meet criteria for employment by Halifax Health

• Demonstrates good interpersonal, oral, and written communication skills
• Providing nursing care to pediatric, adolescent, and geriatric patients
• Works under the direction of a Registered Nurse
