We are seeking a compassionate and skilled Licensing Practical Nurse (LPN) to join our team at Eastern Star Home, a opportunity with a competitive hourly rate of $30 - $33.5.

As a member of our healthcare team, you will play a vital role in providing exceptional care and support to our residents. Your responsibilities will include:

• Providing direct patient care, including administering medications, wound care, and performing vital signs
• Developing and implementing individualized care plans in collaboration with our interdisciplinary team
• Monitoring and reporting changes in residents' conditions to the nursing staff and administration
• Participating in quality improvement initiatives to ensure continuous improvement in patient care
• Collaborating with other departments, such as rehabilitation and social services, to ensure comprehensive care
• Maintaining accurate and timely records of patient care

• Current LPN license in the state of New York (or ability to obtain within [insert timeframe])
