Home Care Registered Nurse - Per Diem

We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills. This role is essential in ensuring that patients receive the necessary care and support in a comfortable environment, promoting their overall well‑being and recovery.

We are looking to hire a Registered Nurse to join our team to conduct in‑home assessments in Orange and Dutchess Counties.

This is a per‑diem position, based on the amount of assessments available that month. No minimum amount of hours or visits is guaranteed.

The RN will work under the direction of the DPS (Director of Patient Services) to evaluate clients in their homes.

These visits are to assess the client’s home‑care needs, create or adjust a Plan of Care, and ensure the caregiver is implementing the plan appropriately.

The RN instructs and advises patients, families, and home‑health aides in their care.

Reassessments are assigned at the beginning of each month and should be scheduled by the nurse according to both her and the client’s availability.

A list of aide annual supervisions is assigned at the beginning of the month.

Start‑of‑care assessments occur on a non‑predictable schedule but must be completed within 24 hours of the start of care.

PRN visits are required if there is a change of condition that warrants a nurse’s follow‑up.

The nurse conducts in‑office orientations for all new caregivers in a group setting, on a predetermined schedule, at our Newburg office.

The nurse is responsible for submitting all documentation from visits to the DPS within 24 hours of the visit.

The nurse must communicate any concerns or client changes to the DPS.

Occasionally the nurse may conduct visits in Rockland County when the Rockland nurses are unavailable.

• Valid and current New York State RN license and registration.
• Active driver’s license and reliable transportation.
• Clear English.
• Strong assessment skills.
• Able to work independently and as part of a team.
• Good oral and written communication skills.
• Previous home‑care or assessment nursing experience preferred.
• Updated physical exam.
• Up‑to‑date with all immunizations.
• PPD.
