About This Role

The Home Care Liaison RN coordinates and directs homecare services with physicians, nurses, and hospital discharge planning staff to determine patient appropriateness for homecare. This role is ideal for RNs looking to make a significant impact in patient care within the home setting.

• Coordinate homecare services with physicians and nurses.
• Direct hospital discharge planning for homecare appropriateness.
• Collaborate with healthcare teams to ensure patient needs are met.

• Graduate of an accredited RN program.

• Previous experience in a certified home health agency.

• License and current registration to practice as a registered Professional nurse in New York State.
