Andrus on Hudson is looking for compassionate and dedicated Licensed Practical Nurses (LPNs) to join our exceptional nursing team. What You'll Do- Provide direct nursing care to residents in accordance with their individualized care plans
- Administer medications and treatments as ordered and in accordance with applicable regulations and facility policies
- Monitor residents’ conditions and promptly identify and report changes to the appropriate nursing staff
- Perform nursing assessments and contribute to the development and implementation of resident care plans
- Communicate effectively with residents, families, physicians, and members of the interdisciplinary care team
- Accurately document nursing care, medications, treatments, and changes in resident condition
- Assist with coordinating and maintaining continuity of care
- Provide guidance and support to Certified Nursing Assistants and other members of the care team
- Promote resident dignity, safety, comfort, and quality of life
- Current New York State Licensed Practical Nurse (LPN) license
- Strong clinical, communication, and organizational skills
- Prior experience using Point Click Care (PCC) or another electronic medical record software
- Compassionate, resident-centered approach to care
- Ability to work collaboratively as part of a multidisciplinary team
- Dependable and committed to providing high-quality nursing care
- Previous experience in skilled nursing, long-term care, rehabilitation, or assisted living preferred
