OASIS Nurse of RN Home Health ICD10 and OASIS

Position Summary

We have an exciting opportunity to join our team as a OASIS Nurse.

In this role, the successful candidate is responsible for analyzing data integrity and consistency of OASIS documentation and assessment processes. The candidate educates/works with clinical staff to clarify documentation and data integrity issues and ensures appropriate coding and sequencing.

• Performs other duties as assigned
• Responsible for detailed review of OASIS clinical assessment documentation to ensure appropriateness, accuracy, completeness and compliance.
• Consults with clinical staff and provides educational guidance on code selection, OASIS data item responses, documentation and Plan of Care development, ensuring documentation reflects the patient's condition and reasons for services.
• Captures the clinical severity for the level of service to ensure quality, cost‑effective care and compliance.
• Works with Agency leadership to address trends affecting outcome and process measures noted during OASIS review.
• Provides education and serves as a resource to clinicians, supervisors, office staff, and billing staff regarding OASIS C requirements, CMS guidelines, home‑care ICD‑10 coding, PPS guidelines and billing issues.
• Assists the Finance Department in creating a Home Health Resource Group (HHRG) and Health Insurance Prospective Payment System (HIPPS).
• Assists Agency leadership in achieving corporate goals, analyzing data, improving patient care outcomes and satisfaction, and participating in quality assessment/performance improvement initiatives.

To qualify you must have a Bachelor’s Degree Nursing, current and valid NYS Registered Professional Nurse license and registration, 1–2 years of Home Health care experience, and working knowledge of CMS, OASIS and ICD‑10 Coding requirements. You should also have knowledge of Federal and NYS Department of Health regulations and The Joint Commission requirements, be able to promote continuous quality improvement concepts, have computer and electronic medical record proficiency, strong communication and interpersonal skills, critical thinking, and meticulous attention to detail.

Registered Nurse License – NYS

OASIS C Certification (COS‑C), ICD‑10 Certification (HCS‑D), and 1–2 years of OASIS review/coding experience in a Home Health agency.

Qualified candidates must be able to effectively communicate with all levels of the organization.
