, part of the Optum family of businesses, is redefining the future of healthcare through a more connected, patient-centered approach. By integrating physical, behavioral, and social care, we're helping individuals access the support they need when and where they need it. As a member of the House Calls team, you'll play an important role in bringing preventive care directly to patients, helping them stay healthier at home while supporting better long-term outcomes.

Join a team that's focused on Caring. Connecting. Growing together.

About the Opportunity

House Calls is a unique, in-home healthcare program that delivers comprehensive wellness assessments to Medicare Advantage and other health plan members. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll travel throughout your assigned area conducting annual health evaluations that help identify care needs, close preventive care gaps, and improve overall health outcomes. This is a field-based, non-prescribing role focused on assessment, education, and coordination with primary care providers.

Key Responsibilities

    • Medical history review and medication reconciliation
    • Physical examinations and vital signs collection
    • Preventive screenings and point-of-care testing when appropriate
  • Identify and document conditions that support care management and treatment planning
  • Share assessment findings with primary care providers to support continuity of care
  • Respond appropriately to urgent or emergent clinical concerns identified during visits
  • Provide education on health conditions, medications, and treatment adherence
  • Connect members with appropriate resources to address social and environmental barriers to care
