About This Role

The Field Nurse in Home Care will deliver safe, therapeutic, population-specific patient care through assessment, planning, implementation, and evaluation. This role is ideal for a compassionate RN with a commitment to quality care and collaboration within a multidisciplinary healthcare team.

• Pay: $65 - $71/hour
• Location: Garden City, NY
• Facility: NYU Langone Hospital—Long Island
• Department/Unit: NYU Langone Hospital--Long Island
• Schedule: Full-Time, Day shift

• Analyzes assessment data to determine diagnoses or issues.
• Identifies expected outcomes for individualized care plans.
• Develops plans that prescribe strategies to attain expected outcomes.
• Implements the identified care plan.
• Coordinates care delivery.
• Employs strategies to promote health and a safe environment.
• Evaluates progress toward attainment of outcomes.
• Practices ethically using the Code of Ethics for Nurses.
• Integrates evidence and research findings into practice.
• Communicates effectively in various formats.

• Education:
  • Bachelor's degree in Nursing required for employees hired after 9/1/2020
• Experience:
  • 1 - 2 years of nursing experience preferred
• Licenses &
  
  Certifications:
  • Current and valid New York State Registered Professional Nurse license
  • Current and valid BLS certification required
