Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 5:
Bohemia, Brentwood, Fire Island, Great River, Islandia, Islip, Islip Terrace, Lake Ronkonkoma, Oakdale, & West Sayville.

• 5x 8 hour shifts (8:30AM to 4:30PM) – Rotating Weekends

• Additional compensation for visits over your caseload

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for patients.
• Measure vital signs, administer IVs, change dressings, clean wounds as needed.
• Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides, and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion directed by physicians’ plan of treatment.

• New York State Licensed Practical Nurse (LPN) license.
• Minimum of one year of recent hands‑on experience.
• Basic Life Support Certification.

USD $33.00 – $40.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
