Care Manager - RN - Inpatient Units

The Care Manager - RN in Inpatient Units at New York-Presbyterian is responsible for providing intensive case management for adult patients with complex medical needs. This role is ideal for a driven and knowledgeable nurse who can lead interdisciplinary teams to ensure quality patient care from pre-admission to discharge.

• Pay: $134,900 - $168,000/year
• Location: New York, NY
• Facility: New York-Presbyterian Allen Hospital
• Schedule: Full-Time, Day shift

• Provide intensive case management for adult patients with complex medical needs.
• Serve as a liaison between the interdisciplinary team and patients/families.
• Direct all aspects of inpatient care and develop individualized care plans.
• Ensure optimal length of stay and help prevent hospital readmissions.
• Anticipate outpatient service needs and set appointments prior to discharge.

Education:

• Bachelor's degree in Nursing (BSN) required

Experience:

• 3 to 5 years of healthcare experience in an acute care setting required
• 2 years of case management/care coordination experience preferred

Licenses &

Certifications:

• Current NYS Registered Nurse (RN) licensure required
