Job Title: Licensed Practical Nurse (LPN) — Skilled Nursing / Long-Term Care
Location: Canandaigua, NY 14424 (Ontario County)
Employment Type: Direct Hire | Full-Time | Part-Time | Per Diem
Pay: $35.00/hr

A skilled nursing and rehabilitation facility in Canandaigua, NY is seeking Licensed Practical Nurses to join its nursing team in a direct hire, permanent role. The LPN delivers direct resident care under the direction of the RN Supervisor / Director of Nursing, administers medications and treatments, documents accurately, and helps supervise nursing assistants on the unit. This is a hands-on floor role in a resident-centered, survey-ready building.

• Administer medications and treatments per physician orders and facility policy; complete accurate MAR/TAR documentation
• Perform routine nursing procedures: wound care and dressing changes, catheter care, ostomy care, tube feedings, glucose monitoring, oxygen therapy, nebulizer treatments
• Complete admission, readmission, and change-of-condition assessments; report findings promptly to the RN Supervisor or provider
• Monitor and document vital signs, intake/output, skin condition, behavior, and response to interventions in the EHR (Point Click Care  or comparable)
• Supervise and direct CNAs on assigned unit, including assignments, break coverage, and follow-through on care plans
• Communicate with residents, families, physicians, pharmacy, and interdisciplinary team members professionally and promptly
• Respond to falls, emergencies, and change-of-condition events; initiate protocols and complete incident documentation
• Participate in care plan meetings, shift report, and infection control, and support survey readiness at all times
• Maintain compliance with NYS Department of Health regulations, CMS requirements, HIPAA, and facility policy

• Active, unrestricted New York State LPN license
• Current BLS/CPR certification (AHA preferred)
• Graduate of an accredited practical nursing program
• Minimum 1 year of LPN experience preferred; skilled nursing/long-term care experience strongly preferred
• IV certification a plus
• Working knowledge of electronic health records;
  Point Click Care  experience preferred
• Physical requirements:
  
  able to lift/assist up to 50 lbs, stand and walk for extended periods
• Must pass background check (including NYS CHRC fingerprinting) and drug screen; TB screening and immunization records required

• Competitive pay of $35.00/hr
