Full Time, Salary Range: $27.00 - $27.00 Hourly, Travel Percentage:
Up to 50% , Licensed Practical Nurse, Foster Care
- Medication management including ordering medications and obtaining prior authorizations
- Communicate with medical director/providers regarding observation in client's health/behavior
- Provide client and family education regarding medications and treatments
- Collaborate with our staff regarding concerns and behaviors
- Participate in Interdisciplinary Medication Review meetings
- Nursing documentation using EHR
- Update discharge planning needs
- Complete the nursing observations report findings to the registered nurse
- Update discharge plnning needs
- See foster children in foster homes as needed
- Other duties as assigned within scope of practice and based on agency needs
- LPN Licensed in New York State required
- CPR Certification required.
- 1 to 2 years of experience
- Experience working with youth and adolescent population desired
- Valid NYS Driver's license
