Registered Nurse (RN) – Infusion & Hybrid Clinical Support

Horizons of Health – Hudson, NY / Clifton Park, NY

Pay: From $40.00 per hour

Schedule: Approximately 4 days per week

Work Arrangement: 2 days in person in Hudson +2 days in person in Clifton Park.

Horizons of Health is a growing integrative medicine practice helping patients with chronic and complex health conditions through personalized, compassionate care.

We are seeking a skilled and dependable Registered Nurse (RN) for a unique hybrid role combining hands-on infusion nursing with remote clinical support for our provider.

This position is ideal for an RN who is confident with IV therapy, enjoys working directly with patients, and is also comfortable supporting a provider with patient communication, prescriptions, labs, prior authorizations, and other clinical follow-up.

The RN will generally work:

• 2 days per week in person at our Hudson, NY clinic performing IV therapy, specialty blood draws, and other clinical responsibilities.
• 2 days per week in person at our Clifton Park, NY clinic performing IV therapy, specialty blood draws, and other clinical responsibilities.
• You will also be supporting the provider with patient portal communications and clinical follow-up.

• Start peripheral IVs with confidence and proficiency.
• Perform specialty blood draws and specimen collection.
• Prepare, administer, and monitor IV therapies according to provider orders and clinic protocols.
• Learn the specialized IV treatments offered by Horizons of Health, including EBOO
  .
• Monitor patients and respond appropriately to infusion-related reactions.
• Educate patients and answer questions within RN scope of practice.
• Maintain accurate clinical documentation in Cerbo
  .
• Maintain a clean, organized, safe, and properly stocked infusion area.
• Collaborate closely with the provider and clinical team.

Once appropriately trained by the provider, the RN will assist with selected clinical responsibilities that can be performed remotely, including:

• Responding to appropriate patient questions through the patient portal.
• Assisting with prior authorizations for labs, medications, and other services.
• Supporting prescription refill and prescription-related requests under provider direction.
• Coordinating laboratory orders, results, and follow-up.
• Assisting with clinical documentation and tasks in Cerbo
  .
• Helping manage selected patient-care responsibilities currently handled directly by the provider.
• Identifying issues that require provider review and escalating them appropriately.

The RN will initially train closely with the provider to learn how patient questions and clinical requests are handled before independently taking on these responsibilities.

• Current BLS certification.
• Strong IV insertion skills are required.
• Experience with specialty blood draws.
• Strong clinical judgment and understanding of RN scope of practice.
• Excellent written and verbal communication skills.
• Strong organization, follow-through, and attention to detail.
• Comfortable working with EMRs, patient portals, and computer-based clinical workflows.
• Able to work independently while recognizing when an issue needs to be escalated to the provider.
• Comfortable working remotely and managing responsibilities without direct supervision.
• Open to learning integrative and functional medicine protocols.
• Reliable transportation to the Hudson clinic
