Pay Rate: $55.00–$60.00 per hour

Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and compassionate Registered Nurse (RN) Supervisor to provide clinical leadership and oversee nursing operations on a per diem, on-call basis
. Coverage includes weekday evening shifts from 4:00 PM to midnight and weekends as needed.

• Oversee and coordinate resident care during assigned shifts.
• Supervise nursing staff and provide clinical guidance and support.
• Monitor resident conditions and respond promptly to changes in health status.
• Collaborate with physicians and interdisciplinary team members to support individualized care plans.
• Ensure compliance with facility policies, procedures, and applicable regulatory standards.
• Promote resident safety, quality care, and respectful communication with residents and families.
• Provide hands‑on nursing care and clinical support as needed.
• Address nursing concerns and assist staff with problem-solving.
• Ensure accurate documentation and effective communication between shifts.

• Active New York State Registered Nurse (RN) license
  .
• Minimum of two years of nursing experience
  .
• Previous supervisory or charge nurse experience preferred.
• Skilled nursing or long‑term care experience preferred.
• Strong clinical judgement, leadership, communication, and organizational skills.
