Join our team at Groton Community Health Care Center as an RN MDS Coordinator!We are seeking a highly skilled and dedicated Registered Nurse (RN) to serve as our MDS Coordinator. As an RN MDS Coordinator at Groton Community Health Care Center, you will play a crucial role in ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. Your primary responsibility will be to identify resident acuity and needs, helping to determine specific care needs and communicating needs and expectations to families and responsible parties.
Responsibilities:- Conduct thorough MDS assessments to identify resident needs and acuity
- Develop and implement individualized care plans in accordance with Medicare requirements and ethical standards
- Collaborate with interdisciplinary teams, including social services, dieticians, rehab specialists, and medical staff, to ensure comprehensive care
- Monitor and evaluate care plans to ensure their effectiveness and make necessary adjustments
- Ensure compliance with Department of Health requirements and regulations
