Our client is seeking a dedicated and proactive Registered Nurse - Assisted Living Facility Nursing Case Manager to join this supportive team.

• Compassionate, empathetic, and patient-centered.
• An excellent communicator, capable of building rapport with residents, families, and staff.
• Proactive in identifying resident needs and developing solutions.
• Skilled in clinical assessment and care plan development.
• Holds an active Registered Nurse (RN) license in New York.
• Must have BSN
