LPN – Nursing

Full Time, 5x8s, with rotating shifts including Days, Evenings, Nights, and Weekends. Holiday work is required.

No specific contract length or guaranteed hours are stated.

Current LPN Licensure in New York

Basic Life Support (BLS) certification from AHA or RQI

Minimum of 1 year of experience as an LPN

Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.

Collect and communicate data related to residents' physical and psychosocial conditions to the healthcare team.

Contribute to the development of care plans that reflect nursing diagnosis and desired outcomes, involving residents and families in the process.

Evaluate resident outcomes and recommend adjustments to care plans as necessary.

Engage in continuous education and mentorship of staff, ensuring adherence to safety and cost-effective practices.

Demonstrate knowledge and competency in nursing practices appropriate for adult and geriatric patients.
