Seven Healthcare are seeking a dedicated Licensed Practical Nurse (LPN) for a full-time D/E rotation assignment in Waterloo, NY. This role requires the provision of skilled nursing care to adult and geriatric patients under the supervision of a Registered Nurse, ensuring patient care aligns with their age-specific needs.

Key Responsibilities of the LPN:

• Collect data on resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Contribute to the creation of age-appropriate and culturally relevant care plans.
• Recommend revisions to care plans after evaluating resident outcomes and collaborate on referrals.
• Maintain nursing practice knowledge and competency, participate in mentoring and orientation of staff.
• Consider safety, cost, and care impact in planning and delivering care, providing leadership under RN direction.

Requirements of the LPN:

• NY LPN Licensure
• BLS Certification (through AHA or RQI specifically)
• 1 year of experience as an LPN
• Experience serving in an LPN capacity at a Long-Term Care (LTC) facility
