Family Specialist

Provide case management services for enrolled families who are k in partnership with enrolled families towards the goal of achieving their maximum potential by learning to recognize obstacles, finding resources, solving problems, and ultimately achieve long-term self sufficiency.

• Performs paperwork and assessment activities; summarizes findings in files.
• Maintains detailed documentation in agency database, CAPTAIN
• Utilize assessments and other forms and systems to identify barriers to success and outline a plan of action.
• Assist families in setting goals and monitoring progress toward goals.
• Incorporate a Trauma Informed Care approach to program delivery.
• Maintains regular contact with assigned families, updates files.
• Maintains contact with area service providers to coordinate services and referrals.
• Refers families to resources, and advocates on family's behalf.
• Facilitates family support groups, meetings, educational opportunities, etc. as needed
• Transports families with agency vehicle when needed.
• Commits agency resources to families for maximum support.
• Participate in staff meetings, case conferences and training as requested by supervisor.
• Works in partnership with other programs to assist families in achieving their goals.
• Understanding of low-income, poverty and diversity-related issues and the Agency’s commitment to the continuum of care and service integration.

• Performs other duties as requested by the supervisor

• Child Development works with families who have children under five:
  • AA/AAS in human services, counseling, social work, family service, or a related field preferred. Or the ability to obtain an AA/AAS within 18 months of hire or a Family Development Credential or an equivalent certification.
  • Experience home visiting, group facilitation, direct support preferred.
  • Must be physically able to perform the duties of the job and lift up to 50 pounds.
  • Microsoft Office applications, internet, email, database entry, and Zoom experience preferred.
  • Ability to relate to and respect people of varied backgrounds
  • Willing and able to travel throughout 3 counties; have reliable transportation
  • Knowledge of local community, state, and federal referral sources
  • Ability to work with minimum supervision and keep detailed accountability
  • Background check, fingerprint clearance, physical exam, and TB
• Family Resources works with families with children of all ages
  • AA/AAS required. AA/AAS in Human Services with experience in human services, family development or related field preferred.
  • Effective oral and written communication skills.
  • Proven ability to develop partnerships with persons from a wide variety of backgrounds.
  • Proven problem-solving ability; self-motivated; ability to work independently.
  • Knowledge of local, state, and federal referral sources.
  • Ability to maintain strict confidentiality.
  • Ability and method to travel throughout Oneida County and the surrounding area.
