Compensation Range: $23/hr - $26/hr commensurate with experience.

OverviewScatter-site Health and Housing Project (SHIP) is seeking a Residential Case Manager to assist with in-home support services to Singles and families aimed to promote independent living within a supportive housing program by providing intensive case management services. The Case Manager will assist residents with entitlements, provide ongoing counseling support, monitor health and wellness and make appropriate referrals, both internally and externally, as needed.
The purpose of this position is to promote independent living within a supportive housing program by providing intensive case management services aimed at developing and enhancing skills of clients in various aspects of their life and assisting clients to be ready for permanent independent housing upon graduation from our program.
- Manages and maintains a caseload of 24 individuals
- Maintains program in contractual compliance by completing individualized treatment plans, assessments, progress notes and other required assessment tools on time and keeps all information confidential.
- Adheres to the provision of service deliveries required in the contracts.
- Submits required reports on time.
- Conducts housing meetings among clients who share an apartment.
- Keeps regular contact with clients including required monthly home visit.
- Maintains agency standards and contractual mandates by following policies and procedures.
- Monitors medical conditions, including viral suppression; of residents and makes appropriate referrals, as needed.
- Helps new residents move into their apartments, including apartment set-up.
- Provides referrals and follow-ups when necessary for substance use, nutrition, clothing, mental health, and dental; said referrals should be internal whenever practical and external as needed.
- Maintains orderly and organized charts.
- Provides accompaniments to client.
- Provides crisis intervention and mediations as necessary.
- Provides backup to other case managers when necessary.
- Provides care coordination to health, mental health, and substance abuse services by utilizing internal and external resources to meet the various needs of clients who were chronically homeless.
- Coordinates multiple enrollments of clients in other Bailey House/ Housing Works programs.
- Obtains updated medical and mental health documentation for each client.
- Assists clients with budgeting, shopping, counseling regarding rent arrears, providing ADL training, and providing orientation to new clients within the program structure.
- Attends weekly supervision, team meetings, and trainings.
- The incumbent is expected to come to supervision prepared with an agenda to discuss casework challenges and successes.
- Travels via the use of mass transit with clients or by themselves in the service of clients.
- Maintains good communication skills to advocate on behalf of clients.
- Contributes to team effort by accomplishing related results as needed.
- Participates in the planning and execution of client activities as determined by program administrators.
- Maintains agency/program operations and workflow.
- Follows agency policies and procedures.
- Attends and participates in weekly supervision regularly, program meetings, and agency and/or funder trainings.
- Maintains professional and technical knowledge.
- Understands and complies with agency guidelines.
- BA in relevant field or BSW with adherence to ethical standards of the social work profession.
- Experience in working with people affected by mental health and/or other chronic conditions.
- Experience working from trauma‑informed care lens.
- At least three years (3) of case management or related experience in housing program operations.
- Proficient in use of electronic health record such as Eicare; ECW, HMIS Microsoft Office (Word, Outlook, Excel).
- Must possess excellent organizational and time management skills.
- Must possess excellent interpersonal skills and be able to work utilizing a team approach.
