Description

Comunilife is looking for experienced Case Managers for its new refugee assistance program opening in Jamaica, Queens this month. The Case Manager is responsible for conducting intakes, initial skills assessments, providing case management and offering referrals for job placements, permanent housing placements or to other appropriate social services based on client need. Provide case management services for new immigrant and refugee clients.

Comunilife offers a generous benefits and paid time off package. The salary range for the Case Manager is $40,000 to $50,000 a year.


Areas of Responsibility:

  • Screens potential clients for program enrollment and orients clients to the program, describing program objectives.
  • Conducts initial intake assessments and utilizes assessment tools (CARES) to identify support service needs, making immediate referrals.
  • Provides assistance to clients in the job placement process, including taking clients to interviews, providing job case management, and helping with application completion.
  • Provides assistance to clients in the post-placement case management process, including the ability to retain the job.
  • Tracks and monitors participant progress, overseeing weekly follow-ups via one-on-one sessions. Follows up with participants through mentoring and counseling, meeting with participants to address issues, challenges, and successes to strengthen clients' capacity for long-term self-sufficiency.
  • Maintains confidential case records for all clients.
  • Maintains contacts with outside agencies to refer or obtain additional client services.
  • Conducts collaborative efforts with service providers to maximize service delivery.
  • Facilitates workshops addressing issues related to assisting with career development: skill building, résumé drafting, understanding worker’s rights, and trainings if applicable.
  • Participates in ongoing education and professional development through courses, seminars, and other in-service training in the areas of services for the homeless, cultural diversity training, supportive counseling, and workforce.
  • Provides monthly service outcomes information and supporting documentation to Supervisor for reporting purposes as required by funding agencies.
  • Performs all other duties as requested by managerial staff and other duties as required.
Qualifications


Education and Experience Requirement(s):

  • Bachelor's Degree in Social Work, Public Administration preferred. Associate's degree with at least 5 years of related experience may be accepted.
  • Bilingual Spanish strongly preferred.
  • Good writing skills required.
  • Some customer service and job readiness experience preferred.
  • Knowledge of at least one of the following service areas is required: homeless-related issues, refugee and/or asylum population.
  • Prior training experience in CARES database is helpful.
  • Knowledge of MS Word, PowerPoint, and other database programs.


Knowledge, Skills, and Abilities:

  • Must be able to implement program activities related to required training and follow through independently in accordance with established procedures.
  • Must be able to independently work through routine problems, determining solutions from established standards or procedures.
  • Required on a regular basis to be prepared to utilize appropriate techniques to ensure consumers' safety.
