Geriatric Social Worker

We are seeking Geriatric Social Workers to provide trauma-informed services to older adults, ensuring client safety, dignity, and access to essential resources. This role involves comprehensive assessment, safety planning, case management, and coordination with multidisciplinary partners to address issues of elder abuse, neglect, and exploitation. The ideal candidate is compassionate, detail-oriented, and able to work independently in the field.

• Conduct in-depth assessments of older adults experiencing abuse, neglect, or exploitation
• Evaluate risk factors and develop individualized safety plans
• Perform home visits as needed to gather information and assess client conditions
• Provide supportive counseling, case management, and referrals to medical, legal, housing, and other services
• Assist clients with applications for emergency food cards or financial assistance
• Collaborate with law enforcement, healthcare providers, attorneys, community partners, and Adult Protective Services
• Advocate for client safety, including support with navigating systems such as Family Court for Orders of Protection
• Conduct community outreach to raise awareness around elder abuse
  
  Maintain accurate, timely case notes and documentation in compliance with program standards
• Perform additional responsibilities as required

• LMSW required
• Experience working with older adults or providing case management preferred
• Bilingual skills preferred:
  • English/Spanish
  • English/Russian
  • English/Chinese (ability to read and write in both languages preferred)
• Strong interpersonal and communication skills
• Comfort using online database systems and technology daily
• Ability to work autonomously in the field, problem-solve, and manage clients with complex needs
