Service Coordinator
Community Access is a pioneer of supportive housing and social services in NYC, founded in 1974. Our mission is to advocate for human rights, social justice, and economic opportunities for individuals with mental health concerns. The Service Coordinator role supports participants in our supported housing programs by providing individualized, person‑centered engagement and comprehensive support services.
Base Pay Range$27.80/hr – $27.80/hr
Program / Department DescriptionThe Housing department offers permanent and affordable supportive housing, transitional residential programs, and services that help residents maintain stability and advance their life goals.
Position OverviewThe Service Coordinator facilitates engagement and support for program participants, helping them maintain housing, set and reach goals, access treatment and community services, build social networks, and explore educational or vocational opportunities. The role also involves leading community‑building group activities.
Key Performance Indicators- Facilitate a minimum of four groups per month (either four different groups or two recurring groups).
- Complete all assessments and service plans on time.
- Enter at least two service‑plan linked notes for all tenants on the caseload within 48 hours of meetings.
- Conduct monthly in‑apartment visits for all tenants on the caseload (weekly in community residences).
- Document 40 hours of direct service work each month.
- Understanding, appreciation, and commitment to the philosophy and mission of Community Access.
- Minimum of a high school diploma or equivalent (GED); bachelor’s degree in a related field preferred (work experience may substitute).
- Ability to prioritize, meet deadlines, and communicate orally and in writing.
- Creative, flexible, and able to use Microsoft Word and Excel.
- Initiative, follow‑through, and independent as well as team oriented.
- Conflict mediation/negotiation skills with an assertive problem‑solving approach.
- Must be fingerprinted and cleared through a background clearance process.
- Bilingual Spanish speaking preferred; additional languages acceptable as needed.
