Pre-Employment Transition Services (Pre-ETS) Counselor

PROGRAM/DEPARTMENT: ACCES-VR/D.V.E. UNIT

REPORTS TO:
Program Manager

JOB LOCATION: 151 Lawrence Street, Brooklyn, NY 11201

WORK HOURS:
Full time - 35 hours Monday - Friday

Salary Range: $50,000.00 to $50,000.00

Position Summary:

The Pre-ETS Counselor is responsible for delivering high-quality, individualized Pre-Employment Transition Services to Students with Disabilities (SWDs), in accordance with ACCES-VR guidelines. The counselor will support students in preparing for employment, exploring postsecondary education and career options, developing self-advocacy and soft skills, and obtaining meaningful work experiences. Services may be provided individually or in group settings, with an emphasis on each student’s unique needs and goals.

Responsibilities:

Assess students’ strengths, interests, and needs to guide service delivery and goal-setting.

Deliver services both one-on-one and in group format, adapting content to suit the learning style and developmental level of each student.

Maintain accurate records of services provided in Apricot and other databases

Collaborate with school personnel, families, community providers, and ACCES-VR staff to support a coordinated transition process.

Monitor student progress and adjust service delivery as needed to align with changing needs and goals.

Adhere to all program, agency, and state requirements related to confidentiality, documentation, and reporting.

Deliver Pre-ETS services in individual or group settings, adapting content based on student needs and developmental levels.

Maintain detailed service documentation and ensure billing aligns strictly with direct service time per student.

Utilize an ACCES-VR approved curriculum to provide Core Rehabilitation Services (CRS) & Pre-Employment Transition Services (Pre-ETS) to individuals with disabilities.

Assess, plan, and provide services to eligible individuals with disabilities

Effectively manage the documentation of monthly service reports and data entry in alignment with ACCES-VR

Complete ACCES-VR service reports within 10 days of completion of services

Develops and maintains relationships with vendors, community partners, and businesses related to opportunities for individuals with disabilities

Engages in outreach to potentially eligible students with disabilities, schools, and community partners to obtain referrals

Meet with referral sources, families, and other service providers to discuss and implement strategies for vocational success.

Complete other duties as assigned

Qualifications:

Bachelor’s degree in Rehabilitation Counseling, Education, Psychology, Social Work, or a related field.

Experience working with youth with disabilities, especially in transition-related services.

Knowledge of vocational rehabilitation systems, transition planning, and disability services.

Strong organizational, communication, and interpersonal skills.

Ability to work independently while managing multiple students and service plans.

Able to work in collaboration with program staff and outside sources.

Must undergo pre-employment screenings such as the Criminal Background Check required by OPWDD
