Full-time Description

The Intake Case Manager plays a vital role within GMHC’s Housing team, supporting individuals and families experiencing housing instability, including those with histories of homelessness. This position is responsible for coordinating comprehensive intake assessments, facilitating referrals to critical services, and supporting clients in accessing entitlements, healthcare, and permanent housing solutions.

With a caseload of up to 25 clients, the Intake Case Manager serves as a consistent advocate and point of contact, providing person-centered support that promotes housing retention and overall well-being. This role involves close collaboration with internal departments and external partners, rigorous documentation and data entry, and an unwavering commitment to equity, dignity, and the lived experiences of our clients.

Essential Job Functions

The Intake Case Manager will be responsible for the following duties:

Intake & Assessment

  • Collaborate with HRA to verify client eligibility and ensure enrollment in GMHC services.
  • Conduct comprehensive intake assessments to determine needs and eligibility for public benefits, entitlement, and supportive housing.
  • Conduct and complete reassessments every six months.
  • Guide clients through housing search and placement processes, including conducting apartment viewings and assisting with related documentation.
Case Management & Support Services

  • Caseload Management:
    Maintain a caseload of up to twenty-five (25) clients across the five boroughs of New York City, providing tailored support and consistent monitoring to promote housing stability.
  • Home Visits:
    Schedule and conduct at least one monthly home visit per client to assess housing stability and provide case management services.
  • Develop and implement individualized case management plans based on intake assessments, providing ongoing support, counseling, healthcare coordination, and entitlements assistance to promote client well-being.
  • Monitor client progress and adjust plans as necessary to meet housing stability goals.
  • Support clients in completing applications and renewals for housing and utility subsidies, advocating for access to the financial assistance they’re entitled to.
Housing Stability & Landlord Engagement

  • Educate clients on tenant rights and responsibilities, including lease terms and household management.
  • Coordinate with landlords, housing providers, and property managers to address client needs and prevent housing disruptions.
  • Monitor and collect monthly rent and utility payments where applicable.
  • Conduct monthly apartment inspections with the residence and maintenance teams to ensure continued safe habitability.
Referral Coordination & External Partnerships

  • Maintain the HASA Housing Referral Database, ensuring timely updates on referral outcomes.
  • Connect clients to GMHC programs and external providers based on clients’ goals and needs.
  • Facilitate warm handoffs and ensure follow-through on referrals or service linkages.
Compliance, Reporting & Documentation

  • Maintain accurate and up-to-date client records, including case notes, progress reports, and required assessments, in compliance with program, agency, and funder standards.
  • Ensure timely submission of all documentation and contract deliverables, such as service plans, monthly progress notes, reassessments, home visits reports, and housing inspection forms.
  • Enter data into internal tracking systems (e.g., eCOMPAS, TREAT, AWARDS) and complete time and effort reports as required.
  • Track client progress and housing outcomes using spreadsheets and generate reports to monitor overall program effectiveness.
  • Ensure compliance with all regulatory and reporting requirements, including the HRA-SSA supportive housing census.
Professional Development & Collaboration

  • Stay current on housing policies, benefits programs, and best practices through ongoing training.
