Intake Coordinator

Responsible for coordinating all intake interviews and assessments and maintaining a utilization rate of 90%.

Intake Supervisor

• Complete pre-screenings and intake assessments for clients in need of shelter or other services.
• Review assessments, Psychiatric evaluations and other documents received from referral sources.
• Participate in service and discharge planning and conferencing, when appropriate.
• Complete fieldwork and outreach to build network of potential referral sources.
• Report any suspected abuse or neglect to State Central Register.
• Act as liaison between program sites, community-based organizations, and agencies that serve runaway and homeless youth.
• Complete DYCD Connect (PTS) enrollments for new admissions.
• Work collaboratively with Case Planners and administrative team to ensure quality of service.
• Participate in other program activities including Quality Assurance team.
• Any other assigned tasks.

Bachelor degree in a related field is required, MSW preferred.
