At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk children and families and those with developmental disabilities through quality community based services.
As a Case Planner in our Prevention and Special Medical Program, you ll work directly with families facing serious challenges such as substance use or mental health disorders that place children at risk of foster care placement. You ll guide families through a structured, three-phase treatment model—Initial, Baseline, and Stabilization—designed to improve safety, stability, and overall well-being. Using a multidisciplinary approach, you ll conduct comprehensive assessments, develop service plans, and coordinate interventions tailored to each family s unique needs.
With an average case duration of 12 months, you ll have the opportunity to build meaningful relationships and help families achieve lasting change.
Requirements:
- BSW or bachelor s degree in human services.
- Some knowledge of the child welfare system.
Additional Requirements:
- Fluency in English and Spanish a Plus
- Citywide travel required
