Job DutiesCWS Case Manager TSS performs case management duties for current and former foster young adults (ages 18-21) in the Extension of Foster Care (EFC) Program under the Transition Services and Support Division of Child Welfare Operations in accordance with DSS Policy, Mission and GPS Practice Model. TSS utilizes adolescent and emerging adult development teaming approach, the impact of trauma, and methods/strategies to support young adult’s capacity to thrive and to provide a continuum of care in the areas of life skills development, self-sufficiency, stability, safety, permanency, and well-being.
The role serves as a Coach, Networker, and Advocate for young adults and partners with them using an Adolescent Teaming Approach to prepare for transition to adulthood. Performs on‑call duties.
- Performs case management (contacts, case planning, service navigation) to ensure continued growth, stability & safety, permanency, and well-being. Engages with empathy and respect; provides ongoing assistance, training and transition support. Maintains minimum of monthly face‑to‑face contact, frequent virtual contact, and adjusts based on the young adult’s needs. Documents all activities in CAPSS system per DSS policy.
- Completes, reviews, and updates all assessment tools, including the Child and Adolescent Needs and Strengths (CANS) per DSS policy timelines. Uses CANS to develop an individualized, strength‑based transition plan with the young adult, making timely service referrals and encouraging continuous reflection and adjustment toward self‑sufficiency.
- Provides service navigation by identifying essential community resources, assisting with referral or application processes, and ensuring connection to those services.
- Utilizes family search and engagement strategies to help the young adult build lifelong connections to family and community, locating and engaging absent parents, relatives, and fictive kin for safety planning and support.
- Ensures the young adult and team members are prepared for the Young Adult Transitional Teaming (YATT) to update the Transition and Permanency Plan, assess progress toward goals, and present recommendations to all participants and the Family Court.
- Collaborates closely with legal to ensure federal compliance of EFC enrollment and Title IVE eligibility, conducts six‑month administrative case reviews, and annual permanency hearings. Adheres to court orders, submits court packets timely, prepares the young adult for court participation, and participates in proceedings.
- Maintains child welfare certification, completes 20 hours of yearly training including required certifications, and completes all trainings assigned by HR, SD&T, and supervisor.
- Maintains an active routine of self‑care and seeks support when necessary to minimize secondary trauma to physical and mental health.
A Master's Degree and one (1) year of professional experience in social services, correctional, education, business administration, general administrative, management, or other relevant programs.
A Bachelor's Degree and three (3) years of professional experience in social services, correctional, education, business administration, general administrative, management, or other relevant programs.
Prefer child welfare experience.
