Housing Works is seeking a Patient Care Coordinator in New York to screen patients, verify insurance, and provide medical case management for Primary Care patients.
You will coordinate intake screenings for eligibility across ADHC, OASAS, Health Home, RAD, OMH, and PrEP, perform PRAPARE assessments, and coordinate referrals with the care team. Proficiency in EMR and HIPAA, plus Spanish, are preferred, with flexible hours and a focus on advocacy.
