ACMH, Inc. is seeking a Senior Case Manager/Case Manager for the Bronx Supportive Apartment program. This role provides case coordination and delivers community rehabilitation services to adults with serious mental illness.
The Senior/Case Manager reports to the Program Director and works Monday through Friday from 9:00am to 5:00pm in The Bronx. Responsibilities include home visits, care coordination, housing support, and maintaining client records with a strong emphasis on safety and crisis
