Urban Health Plan in the Bronx, NY is seeking a Social Worker II/III (LMSW) for a full-time, hybrid role with 4 days onsite and 1 day remote. You will provide psychosocial assessments, crisis intervention, and connect patients to needed services in our Integrated Care setting.
Minimum qualifications include an MSW and NYS LMSW certification; bilingual in English and Arabic or Spanish; and experience with crisis intervention. Excellent communication and EHR skills are required.
