Supports for Living is seeking a Care Manager to support individuals transitioning from facilities into the community under the CTI program in Hyde Park, NY.
You will provide intensive case management, coordinate with hospital discharge teams and community providers, and work in a hybrid team office with mileage-reimbursed field visits. The role requires a bachelor's degree and at least 1 year of experience;
Spanish is a plus. Pay is $26.00/hr with a $5,000 sign-on bonus.
