Position OverviewS:
US is seeking a LPN for their adult family shelter in Manhattan.
I. SCOPEOFROLE:
Diagnosisandmanagementofacute,chronicandcomplexhealthproblems;assessing,diagnosingandtreatingillnesses,educatingtheshelterclientsandstaffondiseaseprevention.
Providehealtheducationandcounselingtoindividualsresidingintheshelter.
ESSENTIAL DUTIES&RESPONSIBILITIES:
Meetswith Program Director ,Director of Social Services ,Psychiatrist,Program Nurses,andSocial
Servicestafftocoordinateshelterresidentstoreceiveongoingtreatmentinthecommunity. Supervise Medication Coordinatorandassist Psychiatrist and Psychiatric Nurse Practitionerasneeded . Administermedicationsanddocumentadministrationasrequired. Takevitalsigns,suchasbloodpressure,temperatureandweight Assistwithdiabetesmedicationandtreatment Takemedicalhistoriesandenterinformationincomputersystems Coordinate with Case Manager ,Senior Case Manage rand Director of Social Servicestoensurepropercare ,nutritionandtreatmentplansarefollowed. Followupwithcommunityhealthcareproviderstoensuremedicaltreatmentplansaredocumented,monitoredandfollowed MonitorMARsandmedicationstorage. Workwithresidentsand Case Managersinfillingprescriptionsandensuringrefillsareorderedasnecessary . Ensureconsumersafety,assessconsumerreactionstomedications,assistwithconsumernutritionalneeds. Ensurephysicianordersarecurrent,properlydocumentedandcommunicatedtostaff Monitorclientmedicalequipmentforsafetyandproperfunctioning. Attendandparticipateinregularstafftrainingandschedulingmeetings. Maintainconfidentialityandprivacyregardingallclients’information. Completedailydocumentationofallagencyrequiredmaterial. Completeincidentreportsinaccordancewithpolicyandprocedures. Understandandimplementtheagency’sphilosophy,policiesandmission. Understandandfollowtheagencychainofcommunication. Ensurethesafetyandwell-beingoftheclientsonadailybasis. Workcooperativelywithotherteammemberstocarryouttheprogramgoalsandobjectivesforeachclient. Reportanyactualorsuspectedabuse/neglectinaccordancewithpolicyandprocedure.
Prepareclientsforself-medicationstatusandmonitorasneeded,healthassessmentreferrals. ComplywithallSUSEmployee
Policies,Procedures,and Protocols,SUSCorporate Compliance Policies ,andNYSDept.ofSocial Services Rules ,Regulations,and Statutes. Maintain CPR/First Aid Certification . Assistinbuildingevacuationduringfiredrillsandbuildingemergencies. Referresidentstoemergencycareifneeded,workwithemergencyresponders/emergencyroomstafftocoordinateemergencytreatment,coordinatetreatmentanddischargeplanningwithhospitalstaff. Aschedulethatincludeseveningsandweekendswillberequired.
REQUIRED QUALIFICATIONS & SKILLS
- Completion of accredited program and licensed in New York as an LPN.
PREFERRED QUALIFICATIONS & SKILLS
- Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences. Bilingual preferred.
