Project Food Box is built on a simple belief: food is healthcare.
We’re a Food as Medicine company working with health plans, community organizations, and local farms to deliver medically tailored groceries to people living with chronic conditions and communities facing food insecurity across California and New York. Since 2020, we've delivered nearly 10 million boxes — over 250 million pounds of fresh food.
We're scaling fast and building the systems, processes, and infrastructure to support national growth. The people who join now will shape how we get there.
About the Role
We're looking for a full-time Bilingual Community Navigator to work directly with community members across the Bronx. You'll be out at events several times a week — health fairs, resource fairs, wherever people are. Unlike a hand-off role, you'll take people all the way through: the conversation, the screening, the eligibility assessment, and connecting them to the services they qualify for.
This role requires strong communication skills, attention to detail, and the ability to work effectively with diverse populations in a fast-paced healthcare environment.
What You’ll Do
- Represent Project Food Box at community, outreach, and partner events throughout your territory
- Engage Medicaid members, build trust, and explain the screening and navigation process clearly
- Complete member screenings and eligibility assessments according to program requirements
- Identify member needs, complete referrals, and navigate eligible members to the right services
- Document screenings, assessments, referrals, and outcomes accurately in Hub Spot and required systems, following all privacy and program requirements
- Pass potential partnership opportunities to the team for follow-up
- Provide additional field coverage for larger or priority events when needed
What You Bring
Required
- Bilingual in English and Spanish, with the ability to communicate effectively with Spanish-speaking members and community partners
- Ability to learn and follow Medicaid and Social Care Network program requirements
- Accuracy and consistency in documentation, including handling sensitive member information
- Comfort working independently in the field and traveling throughout the assigned territory
Preferred
- Experience in community outreach, healthcare navigation, Medicaid programs, or social services
- Hub Spot or CRM experience
The expected salary for this role is $27 hourly. Compensation for this role will depend on various factors, including qualifications, skills, and location.
- PTO accrued based on hours worked
- Paid sick leave
- Weekly food box for employees within delivery zones
- 401(k) retirement plan with company match, subject to plan terms and eligibility
We provide reasonable accommodations for applicants with disabilities throughout the application and interview process. To request, please contact hiring .
