Clinical Pharmacist | Inpatient Pharmacy

• Participating in direct patient care through interdisciplinary rounds
• Managing medication therapies according to approved policies
• Monitoring drug efficacy, therapeutic goals, and outcomes
• Ordering and evaluating laboratory tests to support medication management
• Providing counseling, medication history review, and communicating recommendations to providers
• Documenting interventions electronically and serving as a preceptor for pharmacy students and residents
• Providing drug information, fulfilling pharmacy consults, and assisting with projects like MUEs and performance improvement activities
• Reviewing and interpreting provider orders, supervising satellite pharmacy activities, including IV compounding and sterile processing

• Bachelor of Science in Pharmacy or PharmD from an accredited college
• Preferred: ASHP-accredited residency or equivalent experience (3+ years in acute care)
• Board certification preferred
• Current licensure or eligibility in the State of Delaware

Strong clinical judgment, communication skills, ability to work collaboratively within a multidisciplinary team, and leadership in pharmacy operations.
