IJN Health Systems, LLC is a Skilled Home Health Agency, is looking for a Licensed Practical Nurse (LPN) to join our team of strong and compassionate caregivers. Our ideal candidate is a self-starter, motivated, and engaged.

Licensed Practical Nurse (LPN/ LVN) –Home Health Home Care

Responsibilities:

Assess, plan, implement, and evaluate patient care.

Administration of medications and documentation

Health promotion, teaching, and training of family members

Wound care

Patient assessments, coordination of care

Monitor and report changes in patient symptoms or behavior

Qualifications:

L.P.N, LVN or state-specific licensed nurse credential

Must be at least 18 years of age

Previous Home Care experience preferred, and a desire to serve and care for seniors.

Must be licensed and in good standing (LPN) in the state of Delaware

Minimum of six (6) months of experience working with seniors in assisted living, home health, independent living, hospital or long-term care environments and a desire to serve and care for seniors

Ability to react and remain calm in difficult situations

Ability to demonstrate good judgment, decision-making skills, and problem-solving.

Possess written and verbal skills for effective communication and a level of understanding

A driver's License and good access to transportation
