Job Description

The Registered Nurse, RN, is a member of the interdisciplinary team who works together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN is responsible for coordinating care, continuously assessing patients and families' needs, and implementing the interdisciplinary plan of care.

Primary Functions

As a member of the interdisciplinary team, the RN provides comprehensive care to meet the diverse needs of hospice patients and families, ensuring proper assessment, planning, and execution of care plans.

Job Responsibilities

Case management and supervision of nursing staff and volunteers.

Follow VITAS policies and standards, practicing within the scope of the RN license and hospice eligibility requirements.

Report any practice outside of scope to the supervisor.

Supervise delegated nursing tasks to hospice aides, LPNs, VNs, and volunteers.

Perform comprehensive assessments and provide palliation for end-of-life symptoms.

Identify and utilize resources and referrals like PT, OT, dietary, pharmacy, and medical equipment.

Complete timely documentation of care events.

Educate and train patients, families, and caregivers on care and pain management.

Participate in patient care meetings and weekly team discussions.

Provide effective communication and shift reports.

Participate in bereavement support and on-call rotations.

Work with the IDG to develop and follow the patient’s individualized care plan.

Obtain physician orders as needed and evaluate care outcomes.

Delegate nursing tasks appropriately and ensure compliance with care standards.

Document safety assessments for activities outside standard scope, especially in Connecticut.

Deliver primary nursing care for inpatient and continuous care patients.

Participate in Quality Improvement and Performance Improvement Projects.

Develop professional growth goals, attend training, and participate in orientation programs.

Provide excellent customer service and advocate for patients and families.

Handle complaints and conflicts professionally.

Represent the nursing profession positively within VITAS and the community.

Job Requirements

Current RN license in the state of employment.
Minimum of two years’ experience in hospice, home health, or community health in the last five years.
Reliable transportation with adequate insurance and a telephone.

Physical Demands and Qualifications

Must be able to lift over 20 pounds, assist in patient transfers, stand, walk, and perform visual assessments. Reasonable accommodations are available for disabilities.
